Living Well With Diabetes

Living Well with Diabetes will help participants with pre-diabetes manage their health. Topics include techniques to deal with the symptoms of diabetes, fatigue, pain, hyper/hypoglycemia, stress, depression, anger, fear and frustration; appropriate exercise for maintaining and improving strength and endurance; healthy eating, use of medication; and working with healthcare providers. Juniper also offers Programa de Manejo Personal de la Diabetes, a Spanish-language culturally appropriate version of Living Well with Diabetes.

Audience: Adults with pre-diabetes.

Class Requirements
In-Person Class

Duration

  • 2.5-hour sessions
  • 6 sessions, 1 class per week for 6 weeks

Leaders

  • 2 leaders
  • 1 leader should have diabetes
  • Requires a 4-day in-person training or previous certification in a Chronic Disease Self Management Program, plus a 1-day Diabetes Self Management Program cross-training
  • Leaders must teach 1 class per year to maintain program certification

  • 2020 Living a Healthy Life book or 2012 Living a Healthy Life book (English book)
  • Tomando Control de su Salud book (Spanish book)
  • 2 flip-charts
  • 2 easels

  • Recommended 8-16 participants in class
  • Juniper’s average number of participants enrolled in class: 7
  • Juniper’s average completion rate: 73%
  • Completer: 4 out of 6 classes
